National mental health charity, Young Minds has teamed up with Surrey Youth Voice and Surrey Heartlands Integrated Care System in a project to understand young people's experience of a mental health service transition.
This survey is open to any young person aged 17-25 who has received mental health support in Surrey. Feedback will be used to create recommendations to improve the quality of care that young people receive when they go through a mental health service transition. For the context of this survey, a mental health transition is any move between two different services that offer support with mental health.
