The Looked After Children’s team provide initial health assessments and support for children and young people in care placed in and out of Surrey.
We understand that sometimes there can be ‘too much’ information and we want to bring together resources for families and children in one place.
The Looked After Children's team provide a one off medical assessment for Looked After Children (a child who has been in the care of their local authority for more than 24 hours). This is a chance to discuss all aspects of the child's health and wellbeing and make sure that any issues are addressed.
Nurses provide ongoing reviews to make sure that any recommendations from the assessment are being acted on in a timely way so that young people are supported with their health.
The child's social worker will refer them to the Looked After Children's team for a health assessment when they come into care. There is a statutory requirement for this to be completed 20 working days from when a child comes into care.
